Employee Demographic and Contact Information Form Instructions:
1. Complete the Employee Demographic and Contact Information Form with the new changes (e.g. New Address, Phone Number, Email address, etc.)
2. Indicate the type of change at the bottom of the form by checking of the corresponding box
3. Submit to your Supevisor or Human Resources
If you need to change W-4 information like Marital Status or Dependents please also print out the EAA W-4s (See Below). Both these forms only need the bottom section completed, please remember to make sure that you check off if you are single or married, that you included your social security number and that you have signed and dated the form.
